New patient Registrations

Regular Patient Registration

  • GMS1 form (the purple one)
  • Patient Questionnaire
  • Surgery Information Booklet
  • Patient Card (red for men, blue for women)
  • Checklist


  • upon a patient wanting to register, firstly check if they're in our catchment area using the directory & map.
  • Once patient has filled out forms, please check the forms thoroughly whilst patient is still at the desk.
  • We need to check 2 forms of ID, photo ID & proof of address.
  • Inform patient of their new GP you are registering them with
  • check off on the checklist as you go.
  • If patient is on repeat medication, check when they're due to run out & book a telephone appointment for them with a GP (preferrably their new GP). You will need to free type their name into the appointment slot, remembering to change this over on the system once they are registered. Write the appointment time/date on the patient card as once you click off of it, you will not be able to do a search for it.





Registering a child 16 or under

  • GMS1 Form (the purlple one)
  • Childs patient questionnaire
  • Patient card
  • Checklist

(The child is usually registered with the same GP as their mother.)




Registering a looked after child (LAC)

  • GMS1 form (the purple one)
  • childs patient questionnaire
  • LAC (Looked after child) form
  • Patient card
  • Checklist
  • When you are registering a LAC you will need to book them an appointment with a GP (within 2 weeks of being registered) So it's imperative you get them registered ASAP & book the LAC appointment.


Registering a Temporary patient

  • Blue Temporary registration form
  • Ensure they have filled the form out correctly remembering to put the name of their regular GP practice
  • Home address means their address where they usually live
  • Temporary residence address means the address they are staying at temporarily
  • Ensure you find out if it is short stay, long stay or immediate neccassary. Short stay up to 15 days, long stay over 15 days up to 3 months
  • Once the form is filled out, write the date of expiry on the top
